For French Nationals: In theory, a French National identification card

For E.U. Nationals, a valid passport is mandatory. For all stays longer than three months, a visa is needed. The same applies to citizens of Australia, Brasil, Switzerland, Iceland, Norway, who may stay a maximum of 3 months with a passport but without a visa.

Citizens of the U.S.A, Canada, Argentina, Bermuda, Brunei, South Korea, Costa Rica, El Salvador, Ecuador, Guatemala, Honduras, Japan, Malaysia, Mexico, Nicaragua, New Zealand, Panama, Singapore, and Uruguay may stay a maximum of one month without visa, but with a valid passport.

Citizens of all other countries (including aliens residing in the U.S.A) must request a visa from the French Embassy or French Consulate near their place of residence. This visa must bear the mandatory mention: "Valable pour la Polynésie Française" (Valid for French Polynesia).
No special vaccination is required to travel to French Polynesia.
Customs
Any non prohibited item brought by a tourist for personal use may be entered duty free if it is re-exported within 6 months. Some products such as tobacco, alcoholic beverages, perfumes, coffe and tea, are permitted only in limited quantities. Travelers are allowed to bring in their luggage and for personal consumption:

200 cigarettes or 100 cigarillos or 50 cigars, 2 liters of liquor, Champagne or wine, 50g of perfume, 1/4 liter of eau de toilette, 500g of coffee and 100g of tea.
For any other merchandise, an allowance of 30,000CFP (or 250 Euros) per traveler over 15 years old is granted (125 Euros if younger than 15). Travelers under 17 do not benefit from alcohol and tobacco allowances.
Any telecommunication or radio device requires an importation authorization. Information is given by the Post and Telecommunication Department of the Office of the High Commissioner of French Polynesia. Strictly prohibited importation of: live animals, any plants (except if accompanied by an international insect treatment certificate), flowers and fruits (except with special authorization), counterfeit and cultured pearls or pearls of Polynesian origin, not to mention of course weapons, ammunitions and illegal drugs.

is enough to enter and stay in French Polynesia. However, in order to transit via the United States, they need, like everyone else, a passport valid until 6 months following their date of return.
